Professional Referrals

How Do I Refer a Patient?

If you are a professional wanting to refer a patient to one of Roxbury’s programs we will need the following information for all potential patients.

  • Demographics, including social security number and date of birth
  • Presenting problem(s)
  • Current services
  • Current medications
  • Pertinent medical history
  • Insurance information

Inpatient Substance Use Disorder Program

Sometimes patients come for admission directly from a community provider or from home and sometimes they transfer from other hospitals.  We understand that as a community provider you may not have all of the information we need.  If any of the below information is not available our admissions department will gather as much information as possible to determine eligibility for admission.

  • Current use to include substances used, how they are being using (PO, snort, smoke, IV), age of first use, duration/frequency of current use, last use, and amount used at last use
  • Previous treatment including places and dates
  • Psychiatric evaluation
  • Health and Physical
  • Recent labs
  • Urine Drug Screen

Inpatient Psychiatric Program for Adults and Adolescents

  • Recent labs
  • Urine Drug Screen
  • Pregnancy test (if applicable)
  • Commitment paperwork (either voluntary or involuntary)
  • Information on custody arrangements (for adolescents)

Please call 717-530-2206 to provide a referral over the phone, or fax the referral information to 1-717-532-2109. If you choose to fax referral information, please call and provide admissions with the patient name and inform them a fax was sent. Our staff will review and get back to you as soon as possible.

**Please note, for inpatient psychiatric treatment for adolescents, parents/guardians must accompany the child to admission.
